FAQs for Healthcare Professionals

Initially, all referrals should come from a GP or other healthcare professional. In time, people will be able to self-refer if they have Type 2 diabetes. We are also hoping to add self-referral for people living with Type 1 diabetes at a later date. Referrals from primary care teams can be made by dedicated Diabetes Book & Learn referral forms in either DXS, Emis or Vision depending on your local set-up. Referrals from other healthcare professionals should be made via our referral form at www.diabetesbooking.co.uk/SCrefer
As soon as a patient is referred (and assuming you have kindly provided an email address or mobile telephone number) they will almost immediately get an email or message to confirm that they have been referred, signposting them to our booking website. Patients then simply register themselves, create a password and then make a course selection from any course across south London.
Patients who have booked themselves onto a course will receive a confirmation email or message straight away. We also send out timely course reminders too. If a patient has booked via our Diabetes Book & Learn call centre team, and if they don’t have either an email or a mobile number, their course details will be confirmed by post.
All bookings are automatically confirmed to the Course Provider, irrespective of whether the patient has made the booking on the website, or via our Diabetes Book & Learn call centre team. Every Course Provider has access to an online portal, which will show them who is booked onto one of their courses.
The Diabetes Book & Learn call centre team can assist people who do not have an email or a mobile number, as a booking can be made over the phone with them. Please ask patients to call 020 3474 5500 if they have already been referred by a GP or Healthcare Professional.
The booking service is currently available only for people who have been diagnosed with Type 2 Diabetes. However, signposting to courses and programmes for people who are pre-diabetic is available on this website in the Community Support section including The National Diabetes Prevention Programme - a 9 month programme free for those at risk of developing Type 2 diabetes.
